Assertive Community Treatment
The ACT program has achieved its sixth year as a NYSOMH certified evidence-based practice. This multidisciplinary program serves 48 adults across Erie County by providing intensive community based mental health treatment and support services. The professional staff, made up of a psychiatrist, nurse, family specialist, peer advocate, MICA specialist, vocational specialist, and wellness specialist work together to help people who have been referred to the program, because previous treatment attempts have been unsuccessful. This program offers services and skills in the homes of clients, as well as in the community and may include; housing, money management, work assessment and training, personal care/safety, shopping, cooking, medication administration, self-management and general wellness. Hospitalizations and homelessness have been reduced to zero for many participants.
